| Ordering Team UniformsClick to download, print, or save the Team Uniform order form This form can be saved to your hard drive and emailed to our uniform supplier. WSYAA is currently in the process of retooling our uniform ordering process. Many changes will not take place until after the Spring 2008 season has been completed. However, in order to ensure that uniform invoices are paid, we are implementing several new procedures. WSYAA contributes $20 per youth participant that goes toward uniform costs. In order to pay these costs we need a WSYAA Team Roster completed prior to ordering uniforms. This is not the same Roster as the WCRD Team Roster which must be turned in to the Recreation Department. This roster should be completed as soon as green cards are received and emailed to teaminfo@wsyaa.com We must use this WSYAA Team Roster for accounting verification purposes. This Roster must be submitted to WSYAA before ordering uniforms. Once this Roster is complete, you can save the roster to your local computer and print copies for your team's parents, so they will know who their teammates are and have contact information for each player. This information can be copied and pasted to the uniform order form WSYAA has provided. This worksheet also can be saved to your local computer, printed and turned into the uniform vendor. Uniform order information will be verified by the Team Roster. Also, once player names are inputted in to the WSYAA Team Roster, this information can be copied and pasted into the WCRD Team Roster form, which is due at the Rec Department by March 17. After submitting your WSYAA Team Roster by email to teaminfo@wsyaa.com we will provide you with a P.O. # which may be used at Athlete's World to order and purchase uniforms. The coach will be responsible for any amount above the $20 per participant charge and may be responsible for unverified information (if Team Roster is not submitted). The 2008 Season begins April 21, so uniform sizing and selection should take place at one of your first practices in order to assure that you have uniforms back before your first game and by picture day. This year we are considering expanding Picture Day to include a Westside Days Youth Carnival. |
